Age progression family photos in cases where a significant physical change has occurred, usually minor applicants who experience a change since the last time they got a passport AGES 5-15.

Special circumstances

• If one, or both, parents are absent, Form DS-3053: Statement of Consent Version 2020, must be completed by the absent parent(s), the form must be notarized by a U.S. Notary or at another U.S. Embassy or Consulate and mail in original to the parent or legal guardian who will appear with the minor to renew the passport at the embassy. Include 1 legible copy of absent parent(s) IDs. Note that the DS-3053 is only valid for 90 days from the date of signature.
• If one or both parents are deceased, original Death Certificate(s) must be presented, and in the case of both parents, the adult applying on behalf of the minor must present an original or notarized copy of the court order decree where he or she is named as the legal guardian of the minor(s).
• If the mother has lost contact with the father, or vice versa, the applying parent must fill out Form DS-5525 Version 2020. At the time of interview, an officer will review the circumstances and determine its acceptability and waiver of the missing parent’s signature.
