Passport Renewal for Applicants 15 Years of Age or Under
Applicants (children) must appear in person accompanied by both parents (except if only one parent is listed on the birth certificate).

– Passport fee is $135 cash or credit Card
– Passport Card fee (optional) $15
Must present original and copy of the following documents:
- U.S. Birth Certificate or Consular Report of Birth Abroad (CRBA)
- Present Parents’ valid IDs. Examples: Passport from any nationality, Country IDs, Driver’s License, or State ID.
- Child’s most recent U.S. passport: if the child has never held a U.S. passport, documentary evidence of U.S. citizenship must be provided.
- Social Security card (If you never had a U.S. Social Security number, complete and submit this declaration form with your passport application. A parent can sign this form on behalf of a minor).
Additional documents
Age progression family photos in cases where a significant physical change has occurred, usually minor applicants who experience a change since the last time they got a passport AGES 5-15.
Special circumstances
- If one, or both, parents are absent, Form DS-3053: Statement of Consent Version 2020, must be completed by the absent parent(s), the form must be notarized by a U.S. Notary or at another U.S. Embassy or Consulate and mail in original to the parent or legal guardian who will appear with the minor to renew the passport at the embassy. Include 1 legible copy of absent parent(s) IDs. Note that the DS-3053 is only valid for 90 days from the date of signature.
- If one or both parents are deceased, original Death Certificate(s) must be presented, and in the case of both parents, the adult applying on behalf of the minor must present an original or notarized copy of the court order decree where he or she is named as the legal guardian of the minor(s).
- If the mother has lost contact with the father, or vice versa, the applying parent must fill out Form DS-5525 Version 2020. At the time of interview, an officer will review the circumstances and determine its acceptability and waiver of the missing parent’s signature.
First Time Adult Passport Renewals (Age 16 and Up)
Applicants must appear in person accompanied by one parent (ages 16 & 17).
– Complete online and print out single-sided Form DS-11

– Passport fee is $165 cash or credit Card
– Passport Card fee (optional) $30
Must present original and copy of the following documents:

- If your first and only passport was issued when you were an infant or child, you are required to submit family photos through the years to show age progression; we recommend submitting at least one photo per year.

Replacing a Lost/Stolen Passport
Appointment is required
Adults (age 16 and older)

– One Passport photo (White background /2″ × 2″ in size / no eyeglasses / no uniforms) Photo must be different from the one used on previous passport
– Passport fee is $165 cash or credit Card
– Passport Card fee (optional) $30
Must present original and copy of the following documents:
- For children 16 or older: U.S. Birth Certificate or Consular Report of Birth Abroad (CRBA)
- For children 16 or older: Present Parent valid ID. Examples: Passport from any nationality, Country IDs, Driver’s License, or State ID.
- Provide photocopy of lost or stolen passport, if possible.
- Social Security card.
- Police report if the passport was stolen.
Under 16 Years of Age

Special circumstances
Provide travel itinerary to be eligible for an Emergency Passport. These types of provisional passports have a limited validity and are issued on or about the same day they are requested. All appropriate fees apply. You will receive instructions on how to replace a limited validity for a full validity passport at no additional cost.

Replacing Damaged/Mutilated Passport
Appointment is required
Adults in possession of a 10-year validity passport and the damage is immaterial (ie. damage that does not affect the integrity of the passport and the biographical page: cover is washed out or faded, small cuts to one visa page, some spilled liquid on the visa pages, etc.).

– One Passport photo (White background /2″ × 2″ in size / no eyeglasses / no uniforms) Photo must be different from the one used on previous passport

– Passport Card fee (optional) $30
Adults in possession of a 10-year validity passport and if the damage material (ie. severe damage to the book and biographical page: completely washed out, burned, visa pages and stamps ripped off, etc.).

If your emergency passport was issued less than one year ago from the time you submit this application
– Complete online and print out single-sided Form DS-5504
– One Passport photo (White background /2″ × 2″ in size / no eyeglasses / no uniforms). Photo must be different from the one used on previous passport
– There is no fee if using DS-5504 form
– For minor applicants, 15 years old or under, and using the Form DS-5504, only ONE parent is required to sign and submission of original & copy of birth certificate is required
If submitting request more than 1 year after your limited validity passport was issued.
For minors
– Complete online and print out single-sided Form DS-11
– For minor applicants, 15 years old or under, both parents are required to sign.

Under 16 Years of Age

Change Name or Correct Data in Passports
Appointment is required
Change/correction within one year after issuance

– There is no fee if using DS-5504 form

For corrections: Evidence of correct biographic information (original + 1 photocopy)
e.g. Original birth certificate, previous passport.
For name changes: Legal documentation of name change (original + 1 photocopy)

- Social Security Card If you never had a U.S. Social Security number, complete and submit this declaration form with your passport application. A parent can sign this form on behalf of a minor).
- Both parents need to sign the passport application if applicant is 15 years of age or under
- Age family progression photos (photos from baby to actual date)
Important
Possession of a U.S. Birth Certificate does not automatically guarantee a claim to U.S. citizenship.
Examples of additional documents that can be presented at the day of the interview:
– U.S. Social Security card
– Medical and vaccine records,
– School records,
– Photographs,
– Migratory reports for all family members who returned to EL Salvador, bus or flight ticket used to enter El Salvador,
– Deportation papers for parents,
– Consular note from Salvadoran Consulate or Embassy obtained in the U.S prior to departure,
– El Salvador birth certificate or DUI,
– Driver’s license, etc.
